Paid · open source · a fork of Formbricks

When one link
isn't enough.

fooform builds a form in two minutes and it always will, for nothing. fooform Ultra is for when a form stops being a form: surveys that live inside your product, fired at the right people at the right moment, with a team around them and the answers wired into everything else you use.

AGPL-3.0 — you can always run it yourself, for nothing, forever

Two products, one idea

Which one are you?

Most people never need Ultra. That is not false modesty — it is the whole point of the free one.

fooform

You need a form.

A waitlist. A signup sheet. A quick survey for forty people you already know how to reach.

  • You want it live in two minutes
  • One link, sent by hand
  • Answers in a table, exported as CSV
  • Nobody else needs to log in

fooform Ultra

You need to keep asking.

The same question, forever, to the right slice of your users — and the answer needs to land somewhere that matters.

  • Surveys inside your app and on your website
  • Fired on an event, shown to a segment
  • Branching logic and recall across questions
  • A team, with roles — and Slack, Sheets, webhooks

What the money buys

Everything a link
can't do.

Surveys inside your product

A question that appears in the corner of your own app, styled like your app, without shipping a release. The person never leaves what they were doing.

Ask the right people

Trigger on an event — a plan upgrade, a third failed search, thirty days in — and narrow it to a segment. Everyone else never sees it.

Logic that branches

Skip a page, jump to the end, or ask a different follow-up depending on the last answer — and quote what they said back to them.

A team around it

Invite the people who care. Owner, manager, member — projects kept apart, and nobody sharing one password on a sticky note.

Wired into everything

Answers pushed to Slack, appended to a spreadsheet, POSTed to your own webhook the second they arrive. No nightly export to remember.

Yours to keep

Run it on your own hardware in your own country, or let us run it. Either way it is AGPL — the source is public and the exit door is unlocked.

Pricing

Free stays free.

Nothing that works today ever moves behind the paywall. Ultra is a different, larger product sitting next to it.

Credit where it is due

Ultra is a fork of Formbricks.

We did not build an experience-management platform from nothing — we forked an excellent one. Formbricks is an open-source survey platform by Formbricks GmbH, and Ultra is our fork of it with fooform's builder, look and short links grafted on.

The licence

Formbricks' core is AGPL-3.0, which is exactly why this is possible: the AGPL lets anyone run, modify and sell the software. What it does not let anyone do is close it. So Ultra's source is public, our changes are AGPL too, and anyone who uses Ultra over a network can demand the code that served them — including you.

What we left behind

Formbricks keeps its commercial features in apps/web/modules/ee under a separate enterprise licence that forbids selling them. Ultra removes that directory and does not ship, host or charge for anything in it. Everything we sell is AGPL code plus our own work.

What you are paying for

Not the code — you can have that for nothing, above. You are paying for someone else to run it: the servers, the upgrades, the backups, the 3am page, and a person who answers email. That is the whole business.

Formbricks is a trademark of Formbricks GmbH. fooform Ultra is an independent fork and is not affiliated with, endorsed by, or supported by Formbricks GmbH. Read the fork's source →

Early access

Ultra Cloud is
not open yet.

The fork is real and you can run it today. The hosted version is still being stood up — leave your details and you get in first, at launch pricing.

Yes, this is a fooform.

The signup form for the paid tier is built with the free tier. It seemed rude to use anything else.

  • Launch pricing locked for life
  • A month of Ultra Cloud, free
  • No spam — one email when it opens

Before you ask

Fair questions

Is the free version going to get worse?

No. Nothing that works on fooform today moves into Ultra. Ultra is a bigger, different application for a different problem — it is not fooform with features removed. If we ever break that promise, the free one is on GitHub Pages and MIT-adjacent open; fork it and carry on without us.

If it is open source, why would I pay?

Because running it is the expensive part. Postgres, backups, upgrades, TLS certificates, a queue that does not fall over on a Friday. If that sounds like a pleasant weekend, self-host it and pay us nothing — genuinely, that is a supported outcome and the download link is right there.

Can I move from Cloud to self-hosted later?

Yes, and it is the same code, so it is a database restore rather than a migration. Ask and we will hand you a dump of everything — surveys, responses, contacts. There is no export fee and no notice period.

Does the AGPL mean I have to open-source my app?

No. Embedding a survey with the JavaScript widget does not make your product a derivative work — that widget is MIT-licensed upstream. The AGPL applies to Ultra itself: if you modify Ultra and let other people use it over a network, those people can ask you for your modified source. Using it unmodified obliges you to nothing at all.

Why not just use Formbricks directly?

Please do, if it suits you better — they built the hard part and they deserve the money. Ultra exists for people who came in through fooform and want the same builder, the same short links and the same look, with a platform underneath. If you have no attachment to fooform, upstream is the simpler answer.

What happens to a form I already made?

It keeps working, untouched, whether you ever pay or not. Self-contained links do not expire because there is nothing to expire — the form is in the link. Published forms stay on the free tier's dashboard, and Ultra can import any of them from the same JSON the builder already exports.

Start free.
Upgrade if you outgrow it.

Most people never do, and that is fine by us.
